The 'bill' is lubricated and inserted and then the two jaws are moved apart with a screw mechanism, stretching the opening to give a clear view down the throat of the instrument. Objects such as swabs can also be inserted through the throat and into the office. Used by SMers in medical scenes but also in more general use; the ability to slowly increase the tension on the opening is useful in anal fisting as a way of gradually stretching and relaxing the sphincter.
